We followed these step and filed a report

Monday morning an email came from ip-info-request, requesting 'Proof of Ownership, Assignment or Chain of Title for the patent you are applying for evaluation'. We submitted a PDF file of the patent from USPTO website, and a Patent Authorization Form signed by the patent owner. But the following day we received a reply

Thank you for your message. As previously stated, please provide the following information to ip-info-request at amazon.com:

Proof of Ownership, Assignment or Chain of Title for the patent you are applying for evaluation. The attachments you previously provided was not one of those documents

Once we have received this information, we will move forward with our review of your application. Please note further messages that do not provide us with the requested documentation may not receive a response.

Our question is, where can we obtain a template of 'Proof of Ownership, Assignment or Chain of Title for the patent'?
