Did they buy from you on the UK site or the Italy site? What is the value of the order. Answers to both these determine what you have to do. If below £20 (25 Euros) you can offer a returnless refund.
Otherwise, from Seller Help:
Provide a prepaid international return label
Follow these steps to upload a prepaid return label in Seller Central
Select Orders in the drop-down menu and then click Manage returns.
Click Authorise request for the return request you would like to approve.
Under Your return postage label, select I will provide a prepaid postage label for this request.
Upload a shipping label, select the Carrier used and enter the Tracking ID if you have it.
Examples of providers of prepaid labels include: Evri, Royal Mail, UPS, DHL, La Poste
So whichever carrier you choose, go to their website and create and pay for a return label. Save it as a pdf and send it to the buyer.
