Chargeback successful for item not returned

We received notification today of an expensive chargeback that was granted despite the customer not returning the item. The first chargeback was successfully defended by Amazon. The second chargeback we noticed on the chargeback page 2 weeks ago, but the status was: "You are not responsible for this chargeback or its reimbursement.", so we ignored it. Then yesterday we received the email from Amazon that the chargeback was successful and we are being charged. And sure enough, the status has now changed to: "You are responsible for this chargeback, and you funded the buyer reimbursement."

So not only was the chargeback somehow successful without the customer needing to return the item, but now Amazon can change the chargeback status at will weeks after it was "Resolved". This is so incredibly wrong.
